Understanding Feminising Hormone Therapy
Feminising hormone therapy (HRT) uses oestrogen, anti-androgens, and sometimes progestogens to develop physical characteristics aligned with a feminine identity. This guide explains the medications Gender Journey can help you track.

Oestrogen
Oestrogen is the primary hormone in feminising HRT. Gender Journey supports tracking the following forms:
- Estradiol (Oral) — Tablets taken daily. A common starting option, though oral forms pass through the liver.
- Estradiol (Transdermal Gel) — Applied to the skin daily. Bypasses the liver and provides steady levels.
- Estradiol (Transdermal Patch) — Changed every 3–7 days. Provides consistent delivery with minimal daily effort.
- Estradiol Valerate (Injection) — Typically administered every 1–2 weeks. Offers reliable absorption.
- Estradiol Cypionate (Injection) — Typically administered every 1–2 weeks. Another injectable option with a slightly different release profile.
Your healthcare provider will help you choose the form best suited to your health profile, lifestyle, and preferences.
Anti-Androgens
Anti-androgens reduce the effects of testosterone and are often prescribed alongside oestrogen. The app tracks:
- Spironolactone — Oral, taken daily. One of the most commonly prescribed anti-androgens.
- Cyproterone Acetate — Oral, taken daily. Widely used outside the United States (not available in the USA).
- Bicalutamide — Oral, taken daily. A newer option sometimes used as an alternative.
- Finasteride — Oral, taken daily. A 5α-reductase inhibitor that blocks the conversion of testosterone to DHT.
- Dutasteride — Oral, taken daily. Another 5α-reductase inhibitor, similar to finasteride.
Progestogens
Some feminising HRT regimens include a progestogen. Gender Journey tracks:
- Medroxyprogesterone Acetate — A synthetic progestogen.
- Micronised Progesterone — A bioidentical form of progesterone.
The role of progestogens in feminising HRT is an area of ongoing research. Discuss the potential benefits and risks with your healthcare provider.
